Summary Background: Spinal anaesthesia is one of the first procedural skills to be taught to trainees in anaesthesia. Assessment of performance of procedural skills now favors a structured analysis of the necessary competences rather than a subjective opinion of a trainee's skill. Methods: This work has three aims in relation to the procedural skill of spinal anaesthesia: (i) to explore the factors that influence the teaching and learning of the procedure by surveying anaesthetists in two European countries, (ii) to develop a comprehensive hierarchical task and (iii) to construct a valid clinical assessment tool which could be used at the bedside. Results: (i) Anaesthetists in two European countries believe that factors relating to the trainee, trainer and training program are important when teaching the procedural skill of spinal anaesthesia. (ii) A comprehensive hierarchical task analysis detailing over 500 competences necessary for the performance of spinal anaesthesia was constructed. Furthermore the pre-requisite relationship between individual competences was documented. (iii) A valid clinical assessment tool for assessing performance of spinal anaesthesia was devised comprising an eleven point checklist and a global rating scale. Conclusion: The results of our survey can be used to inform future training programs in anaesthesia. The hierarchical task analysis and the clinical assessment tool form the basis of a procedure-specific prototype for assessment of performance of spinal anaesthesia.
